Treatment-resistant depression (TRD) is defined as depression that does not respond to adequate trials of two or more antidepressants from different classes. It affects approximately one-third of people with major depressive disorder, but TRD is far from hopeless — many effective options remain.
Causes of Treatment Resistance
Several factors can contribute to treatment resistance, including incorrect diagnosis (bipolar disorder, for example, requires different treatment), undiagnosed medical conditions (thyroid disorders, vitamin deficiencies), concurrent substance use, medication non-adherence or inadequate dosing, and genetic variations affecting drug metabolism.
Treatment Options for TRD
Medication Strategies: Switching to a different class, augmentation with a second medication, or combining antidepressants
Cognitive Behavioural Therapy: CBT can be particularly effective when added to medication
Transcranial Magnetic Stimulation (TMS): Non-invasive brain stimulation targeting mood-regulating circuits
Electroconvulsive Therapy (ECT): Highly effective for severe TRD, modern ECT is safe and well-tolerated
Ketamine Infusion Therapy: Rapid-acting treatment for acute suicidal depression
Psychedelic-Assisted Therapy: Emerging evidence for psilocybin under controlled conditions
Lifestyle and Adjunctive Approaches
Regular exercise, adequate sleep, dietary improvements, and reducing alcohol can all enhance treatment response. Support groups and peer support also play important roles.

Finding the right treatment for TRD can take time and patience. Each failed trial provides valuable information. Many people who do not respond to multiple treatments eventually find relief through combination approaches or advanced therapies.
